- Minimum Age 16 Years for Filipino Citizens and 18 Years for Foreigners.
- PSA/LCR Authenticated Certificate of Birth for Filipino Citizens.
- Passport and PSA Authenticated Certificate of Marriage for Married Women Applicants.
- Duly Accomplished Student Permit Application Form.
- A Written Consent Letter from the Parent/Guardian and Parent’s Government ID for Applicants Under 18 Years.
- Local Residency Proof, Passport and A Valid Visa with Minimum Six Months Validity for Foreigners.
- Medical Certificate Showing Physical Fitness and Mental Health Condition, Certificate Obtained from an LTO Accredited Clinic.
- Completed 15-Hour Theoretical Driving Course (TDC) and Obtained the Certificate of Completion from an LTO Accredited Driving School or Training Center.
- Tax Identification Number (TIN), if available.
- Must be Able to Read and Write in Filipino or English.
- Must Not Have Any Unsettled Violations.
- Must be Away from Alcohol and Drugs.
